A Clinical Study on Radial Nerve Injury Complicated by Fracture of the Humerus




Abstract
Among complications associated with humeral shaft fracture, the radial nerve injury is probably the most common, but the incidence of radial nerve injury -in humeral ¢¥shaft fracture is relatively low. If examiner is alerts to this neural complication, the signs of the nerve injury vary easily recognizable. Opinions differ on. the neural complication. Some authorities favor early exploration, while the other favor a conservative regime. In general, if there are no signs. of. recovery by the time of fracture. union, the nerve, should be explored. One hundred and eighty five cases of inpatients with fracture of humorous .who had been treated at one university hospital and two general: hospital in Seoul from April 1969 to¢¥ May 1976 were studied clinically and statistically.
The results were as follows:
1) Male was about 2.6 times more predominant than female.
2) The most frequent age distribution was first and second decade and the left humeral fractures were about :1.6 times to the right side.
3) The most frequently encountered fracture site was middle third of humeral shaft (25.0%)
4) Conservative treatment was applied in 58.7% of the cases and open reduction with internal fixation was performed. in 41.3%.
5) Of the patients with humeral fracture, radial nerve injuries were observed in 11. 1% and of the patients with humeral shaft fracture radial nerve involvement were demonstrated¢¥ in 17.9%.
6) The most frequent radial nerve injury was associated with fractures of middle third of humeral shaft.
7) Majority of radial nerve injuries were found in adult (after third decade)
8) Of the 21 cases of radial nerve injury 5.-cases (23.8%) were treated by conservative regime, nerve exploration and open reduction. -were carried out in 7 cases (33.3%), neurolysis after fracture healing was performed in 6 cases (28.6%), injured nerve was repaired in 2 cases (9.5%) and tendon t4ansfer was performed in 2 cases.
